About the Role
We are seeking a highly skilled and compassionate Nurse Specialist to join our team at Calderdale and Huddersfield NHS Foundation Trust. As a key member of our Discharge Lounge team, you will play a vital role in ensuring the smooth transition of patients from our wards to their homes or community care settings.
Key Responsibilities
- Coordinating discharge planning for patients, liaising with care providers, care facilities, and family members to ensure seamless transitions.
- Transferring and accepting patients from various ward areas to the discharge lounge, utilizing your expertise to prioritize patient needs.
- Providing patient education on care providers, discharge plans, medication management, and follow-up arrangements to empower patients and their families.
- Effective communication and documentation on the electronic patient record to ensure accurate and timely information sharing.
- Delivering high-quality, evidence-based nursing care to patients, prioritizing their physical, emotional, and social needs.
Person Specification
- NMC Registered Nurse in a relevant field of practice.
- Practice Assessor/Practice Supervisor qualification or willingness to complete.
- Additional post-registration qualification relevant to the field of practice (desirable).
- Experience working with diverse patient populations, prioritizing workload, and utilizing IT systems.
- Excellent interpersonal and communication skills, with the ability to adapt to meet patient, family, and professional needs.
