About the Role

We are seeking a highly skilled and compassionate Nurse to join our team at Calderdale and Huddersfield NHS Foundation Trust. As a key member of our Discharge Lounge team, you will play a vital role in ensuring the smooth transition of patients from hospital to home.

Key Responsibilities

  • Coordinate discharge planning for patients, liaising with care providers, care facilities, and family members.
  • Transfer and accept patients from various ward areas to the discharge lounge, ensuring a seamless transition.
  • Complete patient education on care providers, discharge plans, medication management, and follow-up arrangements.
  • Provide excellent communication and documentation on the electronic patient record.
  • Ensure patients' hygiene, nutrition, hydration, elimination, and mobility needs are met while in our care.

Person Specification

  • NMC Registered Nurse in a relevant field of practice.
  • Practice Assessor/Practice Supervisor qualification or willingness to complete.
  • Additional post-registration qualification relevant to the field of practice (desirable).
  • Experience of working with patients from diverse cultures and backgrounds.
  • IT literacy and ability to use NHS systems.
