Advanced Clinical Practitioner

We are seeking a dedicated and experienced Advanced Clinical Practitioner to join our team on a temporary basis. In this role, you will conduct comprehensive reviews for patients with long-term conditions, primarily focusing on COPD, asthma, diabetes, heart disease, heart failure, and hypertension.
Key Responsibilities:
- Conduct thorough assessments for patients with long-term conditions.
- Travel to patients' homes for assessments.
- Utilize provided clinical and IT equipment to complete assessments.
- Maintain accurate and detailed patient records.
Requirements:
- Advanced Clinical Practitioner qualification - can be from nursing or pharmacist background.
- Experience in managing long-term conditions (COPD, asthma, diabetes, heart disease, heart failure, hypertension) is preferred.
- Ability to work independently and manage your schedule.
- Excellent communication and interpersonal skills.
