Job Description

Job Title: Trusted Assessor

Job Type: Temporary, Part-time

Location: Huddersfield Royal Infirmary, with some travel to other CHFT and Mid Yorks Trust hospitals

Hours: 30 hours per week, Tuesday to Friday

Job Summary:

We are seeking a skilled and experienced Trusted Assessor to join our team at Kirklees Council. As a Trusted Assessor, you will play a critical role in supporting the discharge system in hospitals across Kirklees, Calderdale, and Wakefield.

Key Responsibilities:

  • Collaborate with partners across the social care and health system to deliver effective timely discharges to Care Homes
  • Work with care home managers to gather information for assessments
  • Reduce waiting times and improve patient outcomes
  • Develop strong relationships with care providers and health professionals
  • Contribute to the development of innovative solutions to improve the discharge process

Requirements:

  • Relevant experience working across the social care and health system
  • Strong understanding of the issues faced by the care sector
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a broader team
  • Flexibility to work in a high-profile and demanding environment
