Older Adults Care Coordinator

The ideal candidate will have experience of working in an OPMH setting and possess excellent communication skills. You will be responsible for coordinating care for older adults, assessing their mental health needs, and developing person-centred care plans.

- Assessing patients' mental health needs and developing care plans.
- Working in partnership with families and carers to ensure patients receive comprehensive support.
Requirements:
- Car driver with business insurance
- Experience of working in an OPMH setting
- Child and Adult safeguarding procedures
Salary: We offer a competitive salary of £30,000 - £38,000 per annum, depending on experience, plus benefits including flexible working options and career development opportunities.
