About the Role

As a Physiotherapist, you will be responsible for assessing, planning, and delivering individualized rehabilitation programs to patients in their homes, community settings, and other locations. You will work closely with patients, carers, and other healthcare professionals to ensure seamless care and support.

Key Responsibilities
  • Deliver high-quality physiotherapy services to stroke patients in the community, including assessments, goal setting, and treatment planning.
  • Work collaboratively with the multidisciplinary team to provide comprehensive care and support to patients and their carers.
  • Develop and maintain effective relationships with patients, carers, and other healthcare professionals to ensure seamless care and support.
  • Contribute to the development and implementation of individualized rehabilitation plans, taking into account patient goals, needs, and preferences.
  • Participate in regular team meetings and contribute to the development of the team's goals and objectives.
Requirements
  • Recognized Physiotherapy Degree Qualification.
  • Health Professions Council Licence to Practice.
  • Valid full UK driving license and access to a vehicle for work purposes.
  • Ability to work flexibly, including weekends and bank holidays.
