Key Responsibilities- Deliver high-quality physiotherapy services to stroke patients in the community, including assessments, goal setting, and treatment planning.
- Work collaboratively with the multidisciplinary team to provide comprehensive care and support to patients and their carers.
- Develop and maintain effective relationships with patients, carers, and other healthcare professionals to ensure seamless care and support.
- Contribute to the development and implementation of individualized rehabilitation plans, taking into account patient goals, needs, and preferences.
- Participate in regular team meetings and contribute to the development of the team's goals and objectives.
- Recognized Physiotherapy Degree Qualification.
- Health Professions Council Licence to Practice.
- Valid full UK driving license and access to a vehicle for work purposes.
- Ability to work flexibly, including weekends and bank holidays.
