Community Recovery Care Coordinator

We are recruiting for an experienced Registered Mental Health Nurse to join our team at HMP Oakwood. As a key member of our Integrated Mental Health and Substance Misuse Team, you will provide expert care and support to individuals with mental health needs, using a recovery-focused approach.
Your responsibilities will include assessing individual needs, developing personalized care plans, and facilitating timely transfers to hospital under the Mental Health Act. You will work closely with our multidisciplinary team to deliver high-quality mental health care, using clinical innovation and evidence-based practice to achieve the best outcomes for service users.
The successful candidate will have a strong background in mental health nursing, excellent communication skills, and a commitment to delivering high-quality care. A competitive salary of £45,739 - £51,471 per annum, depending on experience, plus benefits including pension scheme, generous annual leave, and opportunities for professional development await the right candidate.
