About the Role:
As a Clinical Pharmacist at Virtual Pharmacist Ltd, you will play a pivotal role in enhancing health outcomes for patients with long-term conditions through collaborative care. Job Description:
- Conduct comprehensive medication reviews to optimize patient treatment plans.
- Review and manage acute medication requests with a focus on patient safety and efficacy.
- Efficiently process hospital discharge summaries and clinic letters, ensuring seamless patient care transitions.
- Offer expert support and guidance for patients managing self-limiting conditions, empowering self-care.
- Support practices in achieving Quality Outcomes Framework (QOF) targets, enhancing practice performance and patient outcomes.
- Play a pivotal role in enhancing health outcomes for patients with long-term conditions through collaborative care.
- Undertake additional tasks as needed to support the healthcare team and patient care initiatives.
Requirements:
- GPhC registration.
- Independent Prescriber Status (preferred).
- Minimum of 2 years Post Graduate experience in a GP Practice.
- Proficiency in using SystemOne or EMIS.
- Completion of the CPPE pathway.
- Strong clinical leadership, communication, and interpersonal skills.
