Disability Support Practitioner

We are seeking a skilled and empathetic Disability Support Practitioner to join our team. In this role, you will work with individuals living in their own homes with disabilities and/or additional support needs, providing tailored support and promoting their independence.
Key Responsibilities:- Providing personalized support to individuals with disabilities and/or additional support needs.
- Developing and implementing support plans that meet the unique needs of each individual.
- Working collaboratively with other professionals to ensure individuals receive comprehensive support.
- A salary of £30,000 - £35,000 per year.
- Paid travel during shifts and 45p per mile fuel allowance.
