Community Palliative Care Worker

Trinity Hospice & Palliative Care Services seeks a skilled individual to support patients with a life-limiting illness in a community setting.
The successful candidate will demonstrate strong communication and interpersonal skills to deliver high-quality patient care.
- Provide comprehensive care to patients at the end of life
- Work collaboratively with healthcare professionals to achieve outstanding patient outcomes
- Cultivate strong relationships with patients and their families
- Salary range: £25,000 - £35,000 per annum
- Pro rata annual leave (inclusive of Bank Holidays)
- Health and wellbeing benefits, including flu and Covid-19 vaccinations
