Job Overview

An exciting opportunity has arisen for registered nurses to work within the Community Continence and Urology Service on a full-time permanent basis.

We are seeking enthusiastic individuals who have an interest in continence and urology. You will form part of a well-established team providing specialist care across CWP neighbourhoods and Care Homes.

The main aspect of the role will be to provide continence assessments, implement, and evaluate specialist continence programs of care.

You will participate in the delivery of training to patients, carers, colleagues, and the wider Health/Social Care community.

CWP prides itself on creating a supportive environment for colleagues and students.

The Community Continence & Urology Service provides specialist care to patients, their families, carers, and colleagues across the West Cheshire Footprint.

The successful candidate will join an established team of clinicians, nurses, and administrators.

Main Duties of the Job
  • To carry out continence assessments, implement, and evaluate treatment programs for patients.
  • Professionally responsible for your caseload.
  • Participate in nurse-led continence clinics.
  • Perform clinical skills relevant to the post.
  • Train carers.
  • Responsible for maintaining confidential caseload notes.
  • Work with external agencies to raise the profile of continence promotion.
  • Communicate effectively with relatives, clients, agencies, and the multi-disciplinary team.
  • Act as the patient's advocate at all times.
  • Develop communication strategies to ensure understanding of care programs and concordance with the treatment plan.
  • Encourage good relationships with other statutory and voluntary services through collaborative working.
  • Maintain effective communication within the continence team.
  • Communicate sensitive information effectively, e.g., financial, sexual, interpersonal issues.
  • Manage and prioritize your caseload effectively.
  • Work within budgetary constraints.
  • Participate in risk assessment for patients, carers, and staff.
  • Signpost and advise staff regarding sources of information as needed.
  • Contribute to audit processes where appropriate.
