About the Role

We are seeking a skilled and compassionate Community Practitioner to join our Universal Crisis Team in Northumberland and North Tyneside. As a key member of our team, you will play a vital role in providing high-quality care and support to individuals experiencing mental health crises.

Key Responsibilities
  • Assess and manage risk in complex mental health situations
  • Provide specialist advice and guidance to service users, families, and professionals
  • Work collaboratively with a multidisciplinary team to deliver safe and effective care
  • Develop and implement recovery-based approaches for adults with mental health needs
Requirements
  • Professional qualification and registration as a Registered Nurse, Social Worker, or Occupational Therapist
  • Proven post-registration clinical experience in mental health or a related field
  • Training in CPA, Safeguarding, and MAPPA
  • Up-to-date knowledge of relevant legislation and policies
