About the Role

We are seeking a highly skilled and compassionate Community Practitioner to join our Universal Crisis Team in Northumberland and North Tyneside. As a key member of our team, you will play a vital role in providing specialist advice and support to individuals experiencing mental health crises.

Key Responsibilities
  • Assess and manage risk in complex mental health cases
  • Provide comprehensive assessments of mental health needs
  • Develop and implement effective care plans to support individuals in crisis
  • Work collaboratively with multidisciplinary teams to deliver safe and effective care
  • Provide specialist advice and support to individuals, families, and carers
Requirements
  • Professional qualification and current registration as a Registered Nurse, Social Worker, or Occupational Therapist
  • Proven track record of post-registration clinical experience
  • Training in CPA, Safeguarding, MAPPA, and Child Protection
  • Up-to-date knowledge of relevant legislation
