Community Practitioner

About the Role
We are seeking a skilled and compassionate Community Practitioner to join our Universal Crisis Team in Northumberland and North Tyneside. As a key member of our team, you will work closely with individuals experiencing mental health crises, providing expert assessment, formulation, and management of risk.
Key Responsibilities
- Conduct comprehensive assessments of complex mental health needs
- Develop and implement effective care plans in collaboration with service users, families, and other professionals
- Provide specialist advice on mental health and risk to service users, their families, and other professionals
- Work collaboratively with the multidisciplinary team to deliver safe, effective, responsive, and compassionate care
Requirements
- Professional qualification and current registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist
- Proven track record of post-registration clinical experience
- Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi-Agency Panel for the Protection of Vulnerable Adults), and Child Protection
- Up-to-date knowledge and experience of relevant legislation
