About the Role

We are seeking a highly skilled and compassionate Community Practitioner to join our Crisis Team at Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust. As a key member of our team, you will play a vital role in providing specialist advice and support to individuals experiencing mental health crises.

Key Responsibilities

  • Provide comprehensive assessments and risk management plans for individuals presenting with complex mental health needs
  • Develop and implement effective interventions to support individuals in crisis, including home-based treatment and coordination of Section 136
  • Collaborate with multidisciplinary teams to deliver safe, effective, and responsive care to service users
  • Provide specialist advice and guidance to colleagues and other professionals on mental health and risk management

Requirements

  • Professional qualification and current registration as a Registered Nurse (RNMH, RMN, RNLD) or equivalent as an Allied Health Professional (AHP), Social Worker, or Clinical Psychologist
  • Proven track record of post-registration clinical experience, including training in clinical/psychological interventions and skills relevant to the post
  • Up-to-date knowledge and experience of relevant legislation, including the Mental Health Act
  • Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives
