About the Role

We are seeking a skilled Health and Wellbeing Support Worker to join our team at North Kerrier East Primary Care Network (PCN). As a key member of our team, you will work closely with clinicians, mental health practitioners, and care coordinators to support adults and children across our local population.

Key Responsibilities

  • Manage and prioritise your own caseload to support people in taking proactive steps to improve their physical and mental health conditions.
  • Use coaching skills and motivational interviewing to motivate individuals, their families, and carers to identify their goals for care and activation.
  • Develop and contribute to multi-disciplinary team meetings to effectively manage cases, particularly where there is complexity.
  • Help and support people to access self-management education courses, peer support, or other interventions that support them in improving their health and wellbeing.
  • Support the delivery of personalised care plans by coordinating input from a range of different professionals and services.

North Kerrier East PCN is part of the West Integrated Care Area in Cornwall, providing clinical services and health and wellbeing support services to people living in the towns of Redruth and Camborne, as well as surrounding villages.
