Job Description:

This is a full-time position with a competitive salary of £22,000 - £25,000 per annum. The successful candidate will have excellent communication skills and be able to build strong relationships with our service users and colleagues.

Key Responsibilities:
  • Deliver person-centred care and support to individuals with disabilities and/or additional support needs.
  • Develop and implement individualised care plans in collaboration with our service users and their families.
  • Work collaboratively with our multidisciplinary team to provide holistic support.
