Job Overview

We are looking for a skilled Community Nursery Nurse to join our team in North Wales. As a Community Nursery Nurse, you will play a vital role in promoting the health and well-being of families in our community.

The successful candidate will have excellent communication and interpersonal skills, with the ability to work independently and as part of a team. They will be responsible for facilitating one-to-one, group, and community work that addresses health inequalities and delivers evidence-based practice.

This is a permanent part-time opportunity with a salary range of around £25,000 - £35,000 per year, depending on experience.

Main Responsibilities

  1. Deliver health promotion activities within home, clinic, and other community-based settings as agreed with the Health Visitor.
  2. Contribute to the implementation of the delivery of the Core Program as per Health Board service level agreement and directed by the Health Visitor.
  3. Carry out delegated programs of care or follow up health issues at home or in clinic following initial assessment by HV.
  4. Undertake screening and/or other health-related assessments as and when directed by the Health Visitor.
  5. Refer to appropriate agencies in collaboration with the Health Visitor.
  6. Ensure the delivery of evidence-based practice in order to facilitate effective care.
  7. Work with parents to increase their knowledge and skills to enable them to identify and meet their own and children's health needs.
  8. Work in partnership with the community, statutory, and voluntary agencies in the provision and development of individual and community initiatives.
  9. Undertake drop-in clinics and facilitate groups, e.g., parent and baby groups.
  10. Undertake baby massage sessions and help to organize and lead parenting programs.
