Job Summary

We are seeking a skilled and experienced Advanced Mental Health Practitioner to join our Adult Community Mental Health Team (CMHT) in North Wales. The successful candidate will work closely with our professional team to provide high-quality care to individuals with mental health needs.

About the Role

As an Advanced Mental Health Practitioner, you will be responsible for assessing, planning, implementing, and evaluating care for individuals with complex mental health needs.

Key Responsibilities
  • To assess and develop holistic care plans that meet the unique needs of each client
  • To work collaboratively with multidisciplinary teams to deliver evidence-based interventions and therapies
  • To provide expert guidance and support to family members and carers
  • To contribute to the development and implementation of policies and procedures that promote best practice in mental health care
Requirements and Qualifications

We are looking for an individual who possesses:

  • A current and valid registration with the Nursing and Midwifery Council (NMC)
  • Proven experience working within a mental health setting, particularly in adult community services
  • Strong communication and interpersonal skills, with the ability to build rapport with clients and colleagues alike
  • The capacity to work independently and as part of a dynamic team, adapting to changing priorities and demands
  • An unwavering commitment to delivering high-quality patient care and promoting person-centered approaches
