The Betsi Cadwaladr University Health Board (BCUHB) North Wales is seeking a skilled Mental Health Nurse to work within an established integrated, multi-disciplinary team. The successful post holder will undertake responsibility for the care co-ordination and case management of a defined caseload of clients with complex and challenging needs.

Main duties include ensuring that care and treatment plans are adhered to and that standards are met in line with the Mental Health Measure (Wales). The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Key Responsibilities:

  • To be responsible for the care co-ordination of a designated caseload of clients with severe and enduring mental health problems.
  • To ensure that care and treatment plans are implemented and standards are maintained.
  • To act as a point of contact for GP's and other interface services to offer advice, support and signposting to appropriate agencies.
  • To formulate and develop standard and complex outcome-focused care and treatment plans.
  • To complete comprehensive risk assessments and document findings.
