Prison Resettlement Worker

We are seeking a highly skilled and dedicated individual to join our team as a Through the Gate Worker. As a key member of our Integrated Health and Wellbeing team, you will play a vital role in supporting the resettlement of patients upon release from prison.
Key Responsibilities:- Design and implement holistic resettlement plans to ensure continuity of healthcare for patients upon release
- Work closely with CGL teams, healthcare, prison partners, and external support agencies to coordinate release planning
- Provide advocacy for access to partnership services and contribute to risk management processes
- Support social reintegration, promoting recovery, resilience, peer support, and self-determination
We are looking for an individual with a strong understanding of substance misuse issues and experience working within a similar field, ideally within prison or criminal justice. You will be an excellent communicator, both verbally and in writing, with good IT skills and the ability to work on your own initiative.
