About the Role:

We are seeking a highly skilled and dedicated individual to join our team as a Through the Gate Worker. As a key member of our Integrated Health and Wellbeing team, you will play a vital role in supporting the resettlement and reintegration of patients into the community.

  • Design and implement holistic resettlement plans to ensure continuity of healthcare for patients post-release.
  • Work closely with CGL teams, healthcare providers, and external support agencies to coordinate release planning and ensure effective support for service users.
  • Provide advocacy for access to partnership services and contribute to risk management processes, including the prison ACCT process.
  • Support service users in their social reintegration, promoting recovery, resilience, peer support, and self-determination.
About You:

We are looking for an individual with a strong understanding of substance misuse issues and experience working within a similar field, ideally within the prison or criminal justice sector.

  • Excellent communication and IT skills, with the ability to work effectively with a wide range of professionals and stakeholders.
  • Proactive and able to work on own initiative, with excellent time management and prioritising skills.
  • Good working knowledge of mental health interventions, services, and good practice.
  • Strong interpersonal skills and ability to work in partnership with a wide range of professionals and agencies.
