About the role:

  • Work intensively with patients for 12 weeks prior to and post-release, providing holistic support to facilitate a smooth transition back into the community.
  • Design and implement comprehensive resettlement plans to ensure continuity of healthcare services for individuals upon release from prison.
  • Conduct secondary screening assessments of patients arriving at the prison as part of the Integrated Health and Wellbeing model of service delivery.
  • Collaborate closely with CGL teams, healthcare professionals, prison partners, and external support agencies to coordinate effective release planning.
  • Provide advocacy for access to partnership services, promoting the well-being and rehabilitation of service users.
  • Contribute to risk management processes, including the prison ACCT process as appropriate.
  • Work closely with Through the Gate provision, healthcare services, prison partners, and external support agencies to ensure effective coordination of release planning.
  • Support service users in their social reintegration, enabling them to lead meaningful and purposeful lives, promoting recovery, resilience, peer support, and self-determination.

About you:

  • You will have a strong understanding of substance misuse issues and experience working within a similar field, ideally within the prison or criminal justice sector.
  • Excellent communication and IT skills are essential, with the ability to work proactively and manage time effectively.
  • A good working knowledge of mental health interventions, services, and good practice is required.
  • You will be a supportive team player with strong interpersonal skills, able to work in partnership with a wide range of professionals, agencies, and stakeholders.
  • Ability to manage change successfully, prioritizing the needs of service users.
  • Understanding of information governance processes and commitment to follow necessary safeguards.
  • Opportunities for learning and development, contributing to the growth and improvement of the team.
