Prison Resettlement Worker

We are seeking a highly skilled and dedicated individual to join our team as a Through the Gate Worker. This is an exciting opportunity to make a real difference in the lives of individuals transitioning from prison to the community.
- Work intensively with patients for 12 weeks prior to and post-release, providing holistic support and guidance to ensure a smooth transition.
- Design and implement resettlement plans that address the unique needs of each individual, ensuring continuity of healthcare and support services.
- Conduct secondary screening assessments of patients arriving at prison as part of our Integrated Health and Wellbeing model of service delivery.
- Collaborate closely with CGL teams, healthcare providers, prison partners, and external support agencies to ensure effective coordination of release planning.
- Provide advocacy and support for access to partnership services, promoting recovery, resilience, and self-determination.
- Contribute to risk management processes, including prison ACCT processes as appropriate.
- Work closely with Through the Gate provision, healthcare providers, prison partners, and external support agencies to ensure effective coordination of release planning.
- Support service users in their social reintegration, enabling them to lead meaningful and purposeful lives.
We are looking for an individual with a strong understanding of substance misuse issues and experience working within a similar field, ideally within the prison or criminal justice sector.
- Excellent communication and interpersonal skills, with the ability to work effectively with a wide range of professionals, agencies, and stakeholders.
- Proactive and able to work on own initiative, with excellent time management and prioritizing skills.
- Good working knowledge of mental health interventions, services, and good practice.
- Ability to manage change successfully, prioritizing the needs of service users.
- Understanding of information governance processes and commitment to follow and apply necessary safeguards.
- Commitment to ongoing learning and development, with a desire to improve and broaden professional knowledge and skills.
