About the Role:

We are seeking a skilled and compassionate individual to join our team as a Resettlement Worker. As a key member of our Through the Gate provision, you will play a vital role in supporting individuals during their transition from prison to community life.

Key Responsibilities:

  • Develop and implement comprehensive resettlement plans to ensure continuity of healthcare and support post-release
  • Collaborate with patients, healthcare professionals, and other stakeholders to coordinate effective release planning
  • Conduct secondary screening assessments and provide advocacy for access to partnership services
  • Contribute to risk management processes and support social reintegration
  • Work closely with our team and partners to facilitate successful release planning and ongoing support

Requirements:

  • Proven experience in a similar field, preferably in the prison/criminal justice setting
  • Strong communication and IT skills, with the ability to adapt to change
  • Knowledge of mental health interventions and services, with a commitment to information governance and compliance
  • Collaborative team player with strong interpersonal skills and a passion for professional development
