Position Overview:

The Housing Support Coordinator will play a crucial role in assisting individuals in securing their next stage of housing by providing essential resettlement support, ensuring a smooth transition into their new living arrangements.

The role involves delivering appropriate floating support for a duration of 6 months, ensuring clients are connected to necessary support services for ongoing assistance.

This position requires a client-centered approach, focusing on risk assessment and support planning for a caseload of approximately 10 clients. The aim is to empower clients to maintain their current accommodations while facilitating access to services and activities that enhance their physical and mental well-being as well as life skills.

The Housing Support Coordinator will be responsible for conducting thorough risk assessments to safeguard the well-being of residents and colleagues, adhering to all organizational policies and procedures.

Key Responsibilities:

- Deliver a high-quality, customer-focused service, ensuring all clients are treated with respect and courtesy, in line with organizational standards.

- Advocate for clients as necessary, providing practical assistance to ensure their living spaces are safe and habitable, and accompany them to appointments when required.

- Continuously monitor and safeguard the physical and mental health of residents, collaborating closely with relevant services, including health and social care, to ensure comprehensive support.

- Coordinate the intake process for new residents, including risk assessments and health and safety inductions.

- Proactively manage behavioral challenges within the accommodation, ensuring risks are addressed safely and appropriately, and represent the organization in legal matters when necessary.

Essential Experience:

- Proven experience working with individuals experiencing homelessness and complex support needs.

- Background in recovery-focused and harm reduction service delivery for clients with multifaceted needs.

- Experience in risk assessment, keyworking, and support planning.

- Familiarity with case coordination in a multi-agency environment.

Essential Qualifications:

- Understanding of the effects of Adverse Childhood Experiences and the principles of Trauma-Informed Care when supporting individuals with complex needs.

Additional Information:

- The successful candidate will be required to undergo a background check upon appointment and every three years thereafter while in the role.
