Key Responsibilities:
  • Work closely with customers to complete inductions, risk assessments, and create personalized support plans to address their unique needs.
  • Develop and review support plans that focus on customers' personal health and wellbeing, debt management, benefit entitlement, and move-on housing options.
  • Empower customers to maintain their independence and understand how to manage their own homes.
  • Ensure all properties are well-maintained and health and safety checks are completed.
  • Support customers in developing positive relationships with their support network and other services.
  • Complete administrative tasks in a timely manner, including support plans, forms, and database entries.
  • Support customers in moving on to their own general needs tenancy.
  • Collaborate with the local community to fundraise and raise Concrete's profile in the area.
Requirements:
  • Educated to a GCSE level.
  • 1-2 years' experience in a similar sector or transferable skills.
  • Experience working with customers with complex needs.
  • Excellent communication skills.
  • Understanding of great customer service.
  • Ability to fulfill contractual commitments for commissioned services.
  • Experience supporting financial processes, including expenses and invoice processing.
  • Experience completing forms and applications for benefits and grants.
  • Ability to work under pressure and manage multiple tasks and demands.
  • A motivational approach to supporting customers.
