Community Healthcare Professional

We are looking for a Community Healthcare Professional to join our team in delivering clinical interventions to children and young people in their homes. This is an exciting opportunity to make a difference in the lives of our patients and their families.
Key Responsibilities:- Provide clinical care to children and young people in their homes
- Collaborate with multidisciplinary teams to develop individualized care plans
- Conduct regular assessments and reviews to ensure optimal patient outcomes
Candidates must be a paediatric registered nurse or a registered nurse with additional paediatric qualifications/experience. A car owner with business insurance is essential due to travel requirements.
Salary and Benefits:The salary for this role is £40,000 - £50,000 per annum, depending on experience. You will also receive a comprehensive benefits package, including generous annual leave and access to NHS discount sites.
