We are seeking a skilled and compassionate Community Healthcare Professional to join our team at Leeds Community Healthcare NHS Trust. Located in the vibrant city of Leeds, we provide a range of community-based health services across the entire city.

As a Community Healthcare Professional with our trust, you will have the opportunity to work closely with colleagues from various disciplines, including District Nurses, Community Matrons, Occupational Therapists, Physiotherapists, Neighbourhood Clinical Assistants, Therapy Assistant Practitioners, and Self-Management Facilitators. Together, you will provide holistic care for patients, working collaboratively to co-ordinate the best possible outcomes.

With an estimated salary of £32,000 - £43,041 per annum, depending on experience, you will be part of a strong culture based on deeply-held values, focused on delivering high-quality care to the people of Leeds. Our Adult Services have been rated 'Outstanding' for caring by the CQC, and we offer a range of benefits, including generous annual leave and access to NHS discount sites.

The successful candidate will have excellent communication skills, maintain a high standard of clinical work, and be resilient, motivated, and adaptable to working in an ever-changing environment.
