About the Role

Sunny Acres Skilled Nursing and Rehabilitation is seeking a highly skilled Care Coordination Specialist to join our team. In this role, you will facilitate weekly skilled Medicare meetings and provide clear communication regarding skilled coverage criteria. You will also transmit MDS to CMS and follow up to validate any errors or messages revealed on final validation reports.

Our ideal candidate will have a current RN or LPN license in MA with previous experience in MDS/PDPM (2 years or more) as well as knowledge of general, rehabilitative and restorative nursing and medical practices and procedures. We offer a competitive salary of $65,000 - $85,000 per year based on experience, paid lunch, weekly pay, paid vacation and PTO, health, dental, and vision plus secondary benefit plan options, and 401K plus match.

Key Responsibilities:
  • Facilitates weekly skilled Medicare meetings and provides clear communication regarding skilled coverage criteria
  • Transmits MDS to CMS and follows up to validate any errors or messages revealed on final validation reports
  • Completes PDPM/MDS assessments within regulatory time frames including CAAs
  • Participates and reviews invoices as indicated for skilled patients under consolidated billing
  • Maintains a current PDPM schedule while setting all PDPM MDS ARDs within time frames in the MDS software program
  • Completes MDS accuracy/PDPM reviews with required software/process in a timely manner
  • Participates in Triple check/Compliance reviews
  • Monitors skilled documentation for potential opportunities/changes
  • Maintains and follows a schedule of due dates for all MDS
