Community Rehabilitation Professional

This exciting opportunity has arisen for a Community Rehabilitation Professional to join our Integrated Case Management Team. As a Community Rehabilitation Professional, you will play a key role in providing high-quality, patient-centered care to individuals in their own homes and communities.
You will work closely with other healthcare professionals, including consultants, specialist GPs, nurses, and physiotherapists, to deliver coordinated care and support to patients with complex needs.
To be successful in this role, you will need excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced, dynamic environment.
Responsibilities and Requirements- BSc degree or equivalent in Occupational Therapy
- Registration with the Health and Care Professions Council
- Experience working in community rehabilitation settings
- Ability to work autonomously and as part of a multi-disciplinary team
- A competitive salary of £40,000 - £50,000 per annum
