Job Overview

We are seeking a skilled Occupational Therapist to join our Integrated Case Management Team in the community. The successful candidate will work collaboratively with other teams to provide holistic assessments and treatment plans for patients, aiming to reduce hospital admissions and improve health outcomes.

The ideal candidate will have a BSc degree or equivalent in Occupational Therapy, registration with the Health and Care Professions Council, and experience in community rehabilitation, musculoskeletal outpatients, and adult/older people's rehabilitation.

Key Responsibilities
  • Conduct initial assessments at home, performing holistic evaluations of patients and developing treatment plans in collaboration with consultants.
  • Provide evidenced-based clinical interventions to improve health outcomes and promote patient choice.
  • Work closely with community agencies, including community matrons, district nursing services, and in-patient therapy services.
  • Manage patients holistically in a community setting, working as an autonomous practitioner.

Salary: £35,000 - £40,000 per annum
