Care and Support Coordinator Role

We are seeking a highly motivated and empathetic Care and Support Coordinator to join our team at Sanctuary Group. As a Care and Support Coordinator, you will play a vital role in providing support to individuals who have experienced homelessness, helping them to achieve their goals and live independently.

Key Responsibilities:

  • Assessing customers' needs and developing personalized support plans
  • Providing support and guidance to customers to help them achieve their goals
  • Working closely with external agencies to ensure customers receive comprehensive support
  • Developing strong relationships with customers, ensuring their dignity, choice, and privacy are respected
  • Liaising with colleagues to ensure effective communication and teamwork

Requirements:

  • Experience of working with adults who have experienced homelessness
  • Strong organizational skills, including functional literacy and numeracy
  • Ability to work as part of a team and on own initiative
  • NVQ3 in Health and Social Care, or willingness to work towards
