In this position, you will be responsible for assessing, planning, and implementing patient-specific programmes of care under the Care Programme Approach. You will also act as a CPA Care Coordinator for a case load of patients and provide home treatment when needed.

Your main goal will be to support patients experiencing mental health crises and help them recover from these issues. With your expertise, you will contribute to the continuous assessment of patients and support junior staff members in managing complex cases.
