Are you passionate about providing mental health treatment in the community?

This role offers a unique opportunity to work with patients experiencing mental health crises and support their recovery journey. As a Band 6 Community Mental Health Practitioner, you will assess, plan, and implement patient-specific programmes of care under the Care Programme Approach.

You will also act as a CPA Care Coordinator for a case load of patients and provide home treatment when needed. With your expertise, you will contribute to the continuous assessment of patients and support junior staff members in managing complex cases.

The ideal candidate will have excellent communication skills and be able to work independently as part of a multi-disciplinary team. A speciality lead in Day hospital regarding clients with personality disorder is an added bonus.

**Salary:** £26.50 per hour (NHS capped rates - Band 6)
