About the Job
We are seeking a highly motivated Health Behaviour Change Coordinator to join our team. In this role, you will be responsible for developing and implementing strategies to support individuals in changing their health behaviour.
Key Responsibilities:
- Develop and implement individualised plans to support patients in reducing their medication usage
- Conduct regular assessments to identify areas for improvement and develop targeted interventions
- Collaborate with healthcare professionals to ensure comprehensive care for patients
Requirements:
- Proven experience in substance misuse support or related field
- Knowledge of Cognitive Behavioural Therapy and Motivational Interviewing principles
- Excellent communication and interpersonal skills
