Job Title: Care Coordinator Expert

We are seeking an experienced Care Coordinator to join our team in Colchester. This is a challenging and rewarding role that requires strong communication skills, attention to detail, and the ability to work effectively with a diverse group of people.

About the Role:
The successful candidate will be responsible for assessing the needs of service users, coordinating care plans, and providing support and guidance to patients and their families. You will work closely with a multidisciplinary team to provide high-quality patient care and ensure that patients receive the best possible outcomes.

Key Responsibilities:
- Assessing new referrals and identifying care needs
- Coordinating care plans and implementing evidence-based interventions
- Providing support and guidance to patients and their families
- Working closely with a multidisciplinary team to achieve shared goals
- Maintaining accurate records and reports

Requirements:
- Registration as a Mental Health Nurse (RMN) Part 3
- Good written and spoken English
- Evidence of appropriate CPD post-qualification (desirable)

Salary and Benefits:
This is a full-time position, and we offer a competitive salary of £35,000 - £45,000 per annum, depending on experience. In addition, we provide excellent training opportunities, season ticket loans, NHS discounts, and a range of other benefits.
