About the Role:

Salary: £40,000 - £55,000 per annum, depending on experience
Key Responsibilities:
- To deliver high-quality, patient-centered care to individuals receiving chemotherapy treatment in their homes
- To work closely with multidisciplinary teams to ensure seamless care coordination
- To assess patient needs, develop care plans, and monitor progress
- To maintain accurate records and contribute to quality improvement initiatives
Requirements:
