Key Responsibilities
As a Shift Leader, you will play a critical role in ensuring the successful operation of shifts within our Hospice in Home team. Your key responsibilities will include triaging patient calls 24 hours a day, providing expert guidance and support to patients, and conducting prompt visits when needed. You will work collaboratively with community and primary care colleagues to deliver integrated end-of-life care.

This position involves working varied shifts, including overnight, taking patient calls, and visiting patients as necessary in North East Essex. You will provide exceptional care and support to patients and families through thorough assessments, careful planning, effective implementation, and ongoing evaluation of care needs. The estimated salary range for this position is £46,000 - £56,000.
