Role Overview

We are seeking a dedicated Support Worker to join our HC One team. As a Support Worker, you will play a vital role in delivering person-centered care to our residents.

Duties and Responsibilities

Your key responsibilities will include:

  • Providing personal care and support to residents, including assistance with daily living activities
  • Working collaboratively with healthcare professionals to develop and implement care plans
  • Communicating effectively with residents, families, and healthcare teams to ensure seamless care delivery
Skills and Qualifications

To be successful in this role, you must have:

  • Experience in providing care to vulnerable adults
  • Basic knowledge of dementia care principles
  • Ability to work effectively in a fast-paced environment
  • Strong communication and interpersonal skills
