Care Coordination Manager

Job Summary: We are looking for an experienced Care Coordination Manager to join our team at Service Care Solutions. As a key member of our Adult Services team, you will be responsible for supporting Adults and Carers within the local community.
About the Job:
This exciting opportunity has arisen to join our dynamic team as a Care Coordination Manager. The successful candidate will be responsible for conducting assessments of Adults and Carers, providing advice and support, and promoting the welfare of vulnerable individuals within the community.
Key Responsibilities:
- Assessment and Support: Conduct assessments of Adults and Carers, and provide practical advice, support, and information to help them achieve their goals.
- Community Engagement: Promote the welfare of vulnerable individuals within the community by engaging with local stakeholders, partners, and services.
Requirements:
- Health and Social Care Experience: Previous experience working within Health and Social Care, particularly with Adults and Carers is essential.
- Care Act Knowledge: Demonstrated knowledge and understanding of the Care Act 2014 is required, with a willingness to learn more.
- Relevant Qualifications: A relevant qualification in social or health care, such as an NVQ, is desirable but not essential.
Salary and Benefits:
The estimated salary for this role is £23,040 - £31,200 per annum, depending on experience (£18,720 - £25,920 PAYE). In addition to a competitive salary, we offer a range of benefits including annual leave, pension scheme, and training opportunities.
