We have an exciting opportunity for a skilled Senior Support Coordinator to join our team at Service Care Solutions. As a Senior Support Coordinator, you will play a vital role in supporting Adults and Carers within the local community by conducting assessments, providing practical support, and information, developing support plans, and ensuring effective delivery of services.

The successful candidate will be responsible for:
• Conducting assessments of Adults and Carers in line with relevant legislation, policy, and procedures.
• Providing advice, support, and information to Adults and Carers.
• Collaborating with individuals to develop support plans and arranging/monitoring service delivery and payments.
• Working closely with colleagues and other professional staff to ensure service users' needs are effectively met.
• Monitoring and reviewing services for timely delivery in line with assessed needs.

This role requires previous experience working in Health and Social Care, particularly with Adults and Carers. The successful candidate will also have demonstrated knowledge and understanding of the Care Act 2014, as well as a willingness to learn more. An understanding of the Mental Capacity Act 2005 and other relevant legislation, policies, or procedures concerning Adults is also desirable.

The estimated salary for this role is £23 umbrella an hour (£18 PAYE an hour) for 36 hours per week.
