About the Role

We are seeking a highly skilled and experienced Pharmacist Specialist Homecare to join our team at East Kent Hospitals University NHS Foundation Trust. As a key member of our Pharmacy Homecare Team, you will play a critical role in delivering high-quality pharmacy services to our patients.

Key Responsibilities
  • Provide pharmacy homecare services and clinical pharmacy services in accordance with Trust procedures and priorities set by the Advanced Level Pharmacist-Homecare.
  • Apply all appropriate clinical and governance arrangements as set out in the Royal Pharmaceutical Society (RPS) standards and recommendations in the Hackett Report for Homecare services.
  • Work with the Multi-Disciplinary Team (MDT) to review existing and develop new homecare service programs and specifications.
  • Collaborate with Lead Clinical Pharmacists and outpatients to identify, plan, and develop new opportunities for homecare services.
  • Identify and manage risks in relation to medicines use according to policy/protocol.
About Us

We are a small team based in Kent and Canterbury Hospital, working closely together to offer clinical support services to each hospital within the trust. Our Pharmacy Homecare Team plays a critical role in ensuring patients receive their medicines safely and effectively.

Requirements
  • Registered member of the General Pharmaceutical Council (GPhC)
  • Completion of Foundation stage 1 of PG General Practice Pharmacy
  • Demonstrable knowledge gained through a broad spectrum of clinical pharmacy training and experience
Desirable Qualifications and Experience
  • Postgraduate diploma in General Practice Pharmacy
  • Training and accreditation as a Practice Supervisor or Educational Supervisor
  • Registered with GPhC as an Independent Prescriber

  • Systematic, methodical, attention to detail attributes
  • Excellent communication skills with patients and other healthcare professionals
