Job Title: Apprentice Rehabilitation Worker

We are seeking a motivated and caring individual to join our Primary Care Home Hub team as an Apprentice Rehabilitation Worker. This role is an excellent opportunity to develop your skills and experience in community healthcare, working with patients in their own homes in the Lowestoft area.

Main Responsibilities:
  1. Work with patients to promote self-care and independence, supporting them in managing their daily needs.
  2. Deliver reablement interventions, focusing on activities of daily living, such as dietary and fluid intake management, continence care, and toileting assistance.
  3. Take and record vital signs, including blood pressure, respiration, pulse rate, and oxygen saturation levels, and report any concerns or changes in patient condition to a registered nurse or healthcare professional.
  4. Participate in caring for patients at the end of life, providing emotional support and comfort.
  5. Ensure accurate and timely record-keeping, adhering to ECCH standards.
Requirements:
  • Care Certificate (or willingness to complete)
  • Level II NVQ or equivalent experience in Health or Social Care
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Flexibility to work varied shifts, including weekends and bank holidays
Working Environment:

The Primary Care Home Hub operates 7 days a week, 7:00 to 20:30 hours, and uses SystmOne as a clinical IT system to support mobile working. You will be required to work across different sites and travel to alternative sites as needed.

This role is subject to a Disclosure and Barring Service (DBS) check, and you will be required to undergo the nationally recognised Care Certificate programme on commencing employment.
