Job Title: Apprentice Rehabilitation Worker

We are seeking a motivated and caring individual to join our Primary Care Home Hub team as an Apprentice Rehabilitation Worker. This role is an excellent opportunity to develop your skills and experience in community healthcare, working with patients in their own homes in the Lowestoft area.

Main Responsibilities:
  1. Work with patients to promote self-care and independence.
  2. Support and deliver aspects of activities of daily living, including management of dietary and fluid intake, continence, and toileting.
  3. Take, record, and report vital signs, such as blood pressure, respiration, pulse rate, and oxygen saturation levels.
  4. Recognize and report any changes in patient condition to a registered nurse or healthcare professional.
  5. Ensure high standards of record keeping in line with ECCH standards.
Requirements:
  • Care Certificate (or willingness to complete)
  • Level II NVQ or equivalent experience in Health or Social Care
  • Excellent communication and interpersonal skills
  • Ability to work without direct supervision
  • Reliability and awareness of confidentiality requirements
Working Environment:

The Primary Care Home Hub operates 7 days a week, 7:00 to 20:30 hours. You will be required to work a variety of shifts, including weekends and bank holidays. You may also be required to work with other teams from different locations to cover staff sickness or capacity pressures.
