About the RoleWe are seeking a skilled and compassionate Complex Needs Practitioner to join our team in Telford and Shropshire. As a key member of our outreach team, you will work with individuals who are struggling with gambling-related issues, providing holistic support and guidance to help them achieve positive outcomes.
As a Complex Needs Practitioner, you will be responsible for engaging with clients in a variety of settings, including community outreach and multi-agency partnership settings. You will work closely with clients to identify their needs and develop personalized support plans, addressing issues such as employment, family work, exploitation, physical and emotional health, and domestic abuse.
Key Responsibilities- Provide screening, co-produced assessment, psychosocial interventions, integrated and outcome-focused care planning, after-care support, and onward transition to other services as required
- Manage a caseload of complex cases, completing accurate and timely recording and case management administration
- Work flexibly across the community and within multi-agency partnership settings, on an outreach basis
- Develop and maintain effective relationships with clients, colleagues, and external agencies
To be successful in this role, you will need:
- Significant experience of engaging effectively with clients in an addictions or social care setting
- Knowledge of primary, secondary, and other outreach settings, as well as the functional model
- Excellent communication and interpersonal skills, with the ability to work with diverse client groups
- Good IT skills, particularly Microsoft Office
- A flexible approach, with a willingness to work outside of normal office hours and travel throughout the area
