About the Job:

Responsibilities:

  • Provide direct patient care to adults with mental health needs
  • Assess patients' physical and emotional needs, developing and implementing individualized care plans

  • Maintain accurate and detailed patient records, ensuring timely and effective communication with healthcare teams

Requirements:

  • Active DE license

Salary Range:

$140,000 - $160,000 per year, depending on experience and location.
