Job Summary

We are seeking a highly skilled and experienced Senior Pharmacist to join our team at Great Western Hospitals NHS Foundation Trust. As a Senior Pharmacist, you will play a key role in providing expert support to our Hospital at Home and Urgent Community Response services.

Main Responsibilities
  • Lead and develop the clinical pharmacy service for Hospital at Home and Urgent Community Response, ensuring compliance with national standards.
  • Conduct medicine reviews virtually and face-to-face in patient homes, as necessary.
  • Work as an independent prescriber, optimising medicines and promoting evidence-based and cost-effective prescribing.
  • Utilise clinical assessment skills to support patient reviews within Hospital at Home.
  • Provide professional advice and support during multidisciplinary ward rounds.
  • Assist in managing prescribing across the primary/secondary care interface to promote a unified approach.
  • Provide education and support to patients, carers, and healthcare professionals on medication usage and storage.
  • Support the admission and discharge process to ensure safe transfer of care in relation to medicines.
  • Coordinate the day-to-day activities of the pharmacy service, supervising and line-managing pharmacy technicians and rotational pharmacists.

Person Specification
  • Registration with the General Pharmaceutical Council.
  • Postgraduate qualification in clinical pharmacy or equivalent experiential practice.
  • Independent prescribing annotation or desire to complete qualification.
  • Keeping professionally up to date, including CPD.
