Senior Clinical Pharmacist

We are seeking a highly skilled and experienced Senior Pharmacist to join our team at Great Western Hospitals NHS Foundation Trust. As a Senior Pharmacist, you will play a key role in providing expert support to our services and patients, and will be responsible for leading and developing our clinical pharmacy service.
Main Responsibilities- Lead and develop the clinical pharmacy service for Hospital at Home and Urgent Community Response, ensuring it meets national standards.
- Conduct medicine reviews virtually and face-to-face in patient homes, as necessary.
- Work as an independent prescriber, optimizing medicines and promoting evidence-based and cost-effective prescribing.
- Utilize clinical assessment skills to support patient reviews within Hospital at Home.
- Provide professional advice and support during multidisciplinary ward rounds.
- Assist in managing prescribing across the primary/secondary care interface.
- Provide education and support to patients, carers, and healthcare professionals on medication.
- Support the admission and discharge process to ensure safe transfer of care.
- Coordinate the day-to-day activities of the pharmacy service, supervising and line-managing pharmacy technicians and rotational pharmacists.

Person Specification- Registration with the General Pharmaceutical Council.
- Postgraduate qualification in clinical pharmacy or equivalent experience.
- Independent prescribing annotation or desire to complete qualification.
- Keeping professionally up-to-date, including CPD.
