Main Responsibilities

  • Lead the development and implementation of clinical pharmacy services for Hospital at Home, in line with national standards and guidelines.
  • Conduct medicine reviews virtually and face-to-face, as necessary, to optimize patient care.
  • Work as an independent prescriber, promoting evidence-based and cost-effective prescribing practices.
  • Provide professional advice and support to multidisciplinary teams, patients, and carers on medication management.
  • Assist in the management of prescribing across the primary/secondary care interface, promoting a unified approach.
  • Develop and deliver safe and effective pathways of care for patients with a range of conditions.

Person Specification

  • Registration with the General Pharmaceutical Council.
  • Postgraduate qualification in clinical pharmacy or equivalent experiential practice.
  • Independent prescribing annotation or desire to complete qualification.
  • Keeping professionally up to date, including CPD.
